Athletic Forms Directions
- It must be printed,
completed, signed, and dated by a physician, and then uploaded to your
child’s/children’s Final Forms account
in order to be eligible to try-out or participate in athletics. IMPORTANT:
Please make sure the PHYSICAL EXAMINATION FORM AND MEDICAL ELIGIBILITY FORM
have a doctors SIGNATURE AND DATE OF THE PHYSICAL on them. We cannot accept
them if they are not signed and dated.
- All Siegel
athletic physical forms are now completed through Final Forms which enables parents and
students to complete required athletic forms electronically.
- The only forms that
families will be required to be uploaded to Final Forms are the Physical Examination
Form and the Medical Eligibility Form and Birth Certificate.
- Siegel athletic
department will not accept paper copies of any other athletic forms or packets,
and all required forms must be completed online to be eligible for tryouts and
to participate in athletic programs or activities.
- Parents will
automatically be notified when the child’s forms and physical need to be
updated.
